I want to emphasis the value of what Dino has offered to do. A valid survey and subsequent analysis requires a professional touch and a lot of experience. I work in an organization that thrives on amateur "surveys" and I regard them as worthless. They end up reflecting the hoped for results of the most vocal members of the committee that wrote them.
It's great to collect input from many people about what we want to know but leave the design and analysis to the pro's.
